Job summary

CareHarmony is seeking an experienced Licensed Practical Nurse (LPN/LVN) to join our fully remote care coordination team. You will collaborate with providers to deliver value‑based care, educate patients, and help close gaps in care across the patient journey.

This full‑time role offers a stable weekday schedule with no weekends, remote work from home, and pathways for growth. A multi‑state license and MS Office proficiency are required.

Qualifications

• Active multi‑state/compact license (LPN/LVN).
• Proficient with Microsoft Office Suite.
• Excellent written and verbal communication skills.

Responsibilities

• Manage patient census with a resolution‑driven approach to close gaps in care.
• Identify and coordinate community resources that benefit patient care.
• Provide patient education and health literacy on chronic condition management.
• Perform medication management: identify concerns, reconcile, ensure adherence, and coordinate refills.
• Ensure timely delivery of services such as Home Health and related needs.
• Resolve patient questions and create open dialogue to understand needs.
• Assist in managing referrals and appointment scheduling.
