Job summary

Tuesday Health is a value-based palliative care provider group dedicated to transforming serious illness and end-of-life care. The Complex Care Navigator LPN plays a key role in delivering coordinated, compassionate care for members with serious illnesses.

The LPN collaborates with nurse practitioners and RNs to develop and execute individualized care plans and participates in care rounds to optimize the member experience.

Qualifications

Active and unrestricted LPN license in the State of Ohio.
Experience in clinical/medical setting preferred.
Experience in a multi-disciplinary care model.
Strong communication and organization skills.
Ability to multitask and work under pressure without sacrificing a positive member experience.

Responsibilities

Administer multiple assessments and screening tools within the clinical model with support from nurse practitioners and registered nurses.
Prioritize needs based on assessment results and coordinate with the interdisciplinary care team.
Be accountable for care plan development within the multidisciplinary team.
Participate in care rounds to ensure a coordinated member experience.
Learn and navigate Tuesday Health electronic systems and tools.
