Cadence is actively hiring a Bilingual/Spanish Speaking Clinical Navigator (Licensed Practical Nurse) to provide virtual chronic care support for Medicare patients. This fully remote role entails conducting patient check-ins, coaching, and ensuring patients are engaged in their care. The ideal candidate will have an active LPN/LVN license and at least 5 years of clinical experience.
