Job summary

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. The role emphasizes proactive outreach, coaching, and care coordination in a fully remote, U.S.-based setting.

Qualified candidates hold an active LPN/LVN license with multi-state eligibility and at least 5 years of clinical experience; Spanish fluency is preferred.

Qualifications

• Active compact multi-state LPN/LVN license in the state of residence.
• Minimum 5 years of clinical experience as a Licensed Practical Nurse.
• Remote work readiness with stable high‑speed WiFi.
• An active California LVN license is highly preferred.
• Spanish fluency strongly preferred.
• Experience educating and supporting patients with chronic conditions.
• Familiarity with AI-assisted clinical tools and ability to discuss AI in care delivery.

Responsibilities

• Conduct outbound telephonic patient outreach and one‑on‑one coaching for chronic conditions.
• Coach patients to execute personalized care plans with education on behavior change.
• Assess health status, readiness to change, and lifestyle factors.
• Educate on disease management, medication adherence, and prevention strategies.
• Monitor progress and outcomes via remote monitoring tools.
• Document interactions and coordinate with clinicians and support teams.
• Collaborate in real time using Cadence Platform, Google Workspace, and Slack.
