Job summary

Cadence is seeking a Clinical Navigator (LPN/LVN) to deliver virtual chronic care support for Medicare patients enrolled in Cadence programs. You will conduct patient outreach, provide coaching, and coordinate care with clinicians to close gaps in care.

The role emphasizes documentation in the Cadence platform, adherence to clinical policies, and effective communication with patients to ensure a supportive care journey.

Qualifications

• Active compact multi-state LPN/LVN license required.
• At least 5 years of clinical experience as LPN/LVN.
• Remote work readiness with stable high-speed WiFi.
• Fluency in Spanish preferred.
• Experience with AI-assisted clinical tools is a plus.

Responsibilities

• Provide outbound patient outreach and coaching for chronic conditions.
• Educate patients on disease management and self-management strategies.
• Document interactions and care plans in Cadence platform.
• Coordinate with clinical team to address barriers to care.
• Monitor patient progress and follow-up as part of remote care.
