Job summary

Cadence is seeking a Bilingual/Spanish Speaking Clinical Navigator (Licensed Practical Nurse) to provide virtual chronic care support for Medicare patients. This full-time remote role focuses on proactive patient outreach, care coordination, and engagement for those managing chronic conditions such as diabetes and hypertension.

Qualifications

• At least 5 years of clinical experience as a Licensed Practical Nurse is required.
