Job summary

Cleveland Clinic Health System seeks an LPN Central Admissions to initiate Home Health, Hospice and Infusion Pharmacy services for referred patients. This remote role includes on-site training at the Independence, Ohio center and scheduling Monday–Friday, 9:00am–5:30pm.

You will coordinate initial and final orders, communicate home care needs across scheduling, reimbursement and care teams, and act as the information specialist for the home care services lines.

Qualifications

• Graduate from an accredited Practical Nursing Program.
• Current licensure as an LPN or LVN.
