Cleveland Clinic is seeking a Nursing Admission Specialist in Independence, Ohio, responsible for initiating home care for patients. You will perform comprehensive assessments and develop care plans to support patient independence and reduce hospital readmissions. Qualificações

• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) Certification required.
• Experience in an acute medical/surgical or home-care setting.

• Conduct comprehensive assessments in accordance with OASIS reporting requirements.
• Identify immediate care needs and obtain orders for treatment.
• Obtain consent for care and demonstrate knowledge of coverage criteria.
