You will initiate services for referred patients, supporting care coordination while contributing to clinical operations. Responsibilities include interfacing with nurses and therapists, ensuring timely orders, and guiding families.

Qualifikationen

• Graduate from an accredited Practical Nursing Program.
• Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
• One year of clinical experience in Homecare is preferred.

Aufgaben

• Assess the appropriateness of the requested home care services under the guidance of a Registered Nurse.
• Coordinate initial and final orders on referrals for home care nursing, therapy and social work services.
• Incorporate clinical and financial knowledge with the referral process and recommend appropriate alternatives.
• Communicate home care needs with scheduling, managers, reimbursement and other parties involved in the home care process.
• Serve as the information specialist for the Center for Connected Care Home Care Services.
• Understand services and products available and criteria for service and reimbursement for home care lines.
