Cleveland Clinic Homecare is seeking an experienced LPN to visit patients in their homes across the Akron and Greater Cleveland area. You will provide direct patient care, administer medications within scope, and coordinate with an interdisciplinary health team to ensure quality home-based services.

You will document outcomes, educate patients and families about care options, and support safe discharge planning.

Formación

• Graduate from an accredited practical or vocational nursing program.
• Current Ohio license as a Licensed Practical Nurse.
• One year of clinical experience as a professional nurse in an acute setting.
• Basic Life Support (BLS) certification through the American Heart Association or the American Red Cross.
• Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
• Knowledge of JCAHO and Medicare standards and regulations.

Responsabilidades

• Visit and provide care to approximately 6 patients each day in their homes.
• Administer medications, within scope of practice, and observe effectiveness and adverse reactions.
• Provide direct patient care and dressings, labs, and related tasks as part of the care plan.
• Coordinate discharge planning and communicate with the RN and health team.
• Educate patients and families about care benefits and ensure understanding.
• Maintain up-to-date documentation and record-keeping for patient condition and care.
