Job summary

Cleveland Clinic is seeking an LPN Central Admissions role based in Independence, Ohio. This remote role includes on-site training at the Business Operations Center and a 9:00 am–5:30 pm weekday schedule, with weekends every 5th and two holidays per year.

As part of Home Care and Hospice operations, you will initiate services for referred patients and coordinate orders for home care nursing, therapy and social work while collaborating with scheduling, reimbursement and other teams.

Qualifications

• Graduate from an accredited Practical Nursing Program.
• Current licensure as an LPN or LVN.

Responsibilities

• Facilitate initiation of Home Health, Hospice and Infusion Pharmacy services as referred.
• Coordinate initial and final orders on referrals for home care nursing, therapy and social work services.
• Incorporate clinical and financial knowledge with referral process and recommend appropriate alternatives when needed.
• Communicate home care needs with scheduling, managers, reimbursement and other parties involved.

Education

Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) licensure
