Qualifications

• Graduation from an accredited Practical Nursing program (or equivalent recognized military training).
• Two years of related clinical work experience showing requisite knowledge and abilities.
• Licensed Practical Nurse: WA, upon hire and CPR/BLS certification required.

Responsibilities

• Coordinate and oversee collaborative care for patients referred to Virginia Mason Franciscan Health under physician/ARNP direction.
• Guide patients through intake, diagnosis, treatment, and discharge and ensure referrals and plans are sent to the appropriate provider.
• Communicate with patients, physicians, and outside agencies regarding care coordination and treatment.
• Address treatment plans, referrals, and pre-authorizations to achieve clinical and financial outcomes.
• Complete documentation per hospital policy and perform departmental admin tasks.
