Job summary

Virginia Mason Franciscan Health seeks an LPN Case Coordinator to coordinate patient care under physician/ARNP guidance. You will guide intake, diagnosis, treatment, and discharge, ensuring referrals reach the right specialist and coordinating resources for optimal clinical and financial outcomes.

Must hold a WA LPN license with at least two years of related clinical experience and CPR/BLS certification. Case management experience is preferred; this is a remote position based in Washington state.

Qualifications

• Must be a licensed Practical Nurse in Washington (WA) with two years of related clinical experience.
• BLS/CPR certification required upon hire.
• Preferred case management work experience.

Responsibilities

• Coordinate and oversee collaborative patient care under physician/ARNP direction.
• Guide intake, diagnosis, treatment, and discharge, ensure referrals and treatment plans are sent to appropriate providers.
• Coordinate resources to achieve clinical and financial outcomes; work with community services and agencies.
