Job summary

Virginia Mason Franciscan Health is seeking an LPN Case Coordinator to oversee collaborative care for patients referred to FHS, under physician/ARNP direction, coordinating with providers to ensure guided access to services.

You will guide patients through intake, diagnosis, treatment and discharge, coordinate referrals and treatment plans, and ensure documentation and communication across the care team. Remote position requiring WA residency.

Qualifications

• Graduation from an accredited Practical Nursing program and WA license.
• Two years of related clinical work experience (preferred), upon hire and
• Basic Life Support - CPR certification, upon hire

Responsibilities

• Performs patient intake and discharge activities, including coordinating with the care team.
• Guides patients through intake, diagnosis, treatment and discharge with the team.
• Communicates with patients, physicians, and outside agencies regarding care coordination.
• Addresses and manages treatment plans, referrals and pre-authorization requirements.
• Completes documentation in accordance with hospital policy and supports admin functions.
