Virginia Mason Franciscan Health is seeking a RN Care Coordinator to guide patients through the care continuum, coordinating discharge planning and post-acute services to ensure smooth transitions and high-quality outcomes. This role emphasizes clinical acumen, advocacy, and collaboration with physicians, nursing, insurers, and care teams across the care journey.

Formación

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in WA state.
• BLS/CPR certification within required timeframe.
• Master's degree in Case Management or Nursing preferred.

Responsabilidades

• Completes discharge planning assessment and documents findings.
• Develops and oversees multidisciplinary discharge plan with the care team and post-acute providers.
• Oversees implementation of the discharge plan.
• Collaborates with multidisciplinary team to optimize inpatient resource use.
• Coordinates transitions to appropriate post-acute services based on patient needs.
• Identifies readmission risks and implements mitigation strategies for at least 30 days.
