Virginia Mason Franciscan Health is seeking a Licensed Practical Nurse Case Coordinator to guide patients through intake, diagnosis, treatment and discharge in collaboration with a clinical team.
The role requires two years of clinical work, WA licensure, and CPR/BLS certification, with remote work options and coordination across primary care and specialty services.
As our LPN Case Coordinator, you will coordinate and oversee the collaborative care of patients referred to Franciscan Health System (FHS) under the direction of a physician or ARNP, working with primary care providers such as physicians, pharmacists, diagnostic imaging technicians, and physical/occupational therapists to offer guided access to the full range of services.
Every day you will guide patients through intake, diagnosis, treatment, and discharge, ensure referrals and treatment plans are sent to the appropriate specialist, department, or primary care physician, coordinate resources to achieve desired clinical and financial outcomes, and work with community services and government agencies to support patients' ongoing medical needs.
