Qualifications

Responsibilities

• Manage patient census with resolution-driven approaches.
• Identify and coordinate community resources for patient care.
• Provide patient education on chronic conditions management.
• Perform medication management including coordination of refills.
• Ensure timely delivery of services for patients.
• Assist in scheduling referrals and appointments.
