• Proficient with digital communication tools, EHR systems, and remote monitoring technologies.
• Excellent verbal and written communication skills with empathy.
• Strong organizational and time management skills for remote caseloads.

Responsibilities

• Proactively monitor health and well-being of elderly clients through virtual check-ins and remote monitoring.
