Job summary

TalentBridge partners with a leading national healthcare organization to recruit an experienced Lead Patient Navigator Social Worker who will oversee field-based teams supporting patients receiving specialty therapies.

The role combines people leadership, patient advocacy, reimbursement expertise, and cross-functional collaboration to improve outcomes and ensure successful patient support programs. Remote work with travel required.

Qualifications

• 10+ years of experience in the pharmaceutical or healthcare industry.
• 3+ years of reimbursement management experience.
• 3+ years of direct people management experience.
• Licensed Social Worker (LSW, LMSW, LCSW, or equivalent).
• Bachelor's degree in Social Work, Health Sciences, Healthcare Administration, Business, Public Policy, or a related field preferred.
• Strong leadership, communication, and relationship management skills.

Responsibilities

• Lead, coach, and develop a team of field-based Patient Navigator Social Workers.
• Oversee patient access strategies to meet productivity, quality, and service objectives.
• Partner with patients, caregivers, providers, specialty pharmacies, and internal teams to facilitate access to therapies.
• Provide guidance on insurance coverage, prior authorizations, reimbursement challenges, and financial assistance programs.
• Support coordination of specialty pharmacy services, infusion therapies, and home-based treatment transitions.
• Set and monitor individual and team performance goals and coaching plans.
• Collaborate with leadership to support new therapy launches and improve patient access workflows.
• Maintain compliant patient documentation in CRM systems and ensure privacy requirements.
