TalentBridge is partnering with a leading national healthcare organization to hire an experienced Lead Patient Navigator Social Worker to oversee a team of field-based Patient Navigator Social Workers supporting patients receiving specialty therapies.

This role emphasizes people leadership, patient advocacy, reimbursement expertise, and cross-functional collaboration to improve patient outcomes and ensure successful patient support programs.

Formación

• 10+ years in pharma/healthcare, preferably within specialty pharmacy or patient support programs.
• 3+ years reimbursement management with knowledge of insurance coverage and prior authorizations.
• 3+ years direct people management experience leading teams.
• Licensed Social Worker (LSW/LMSW/LCSW) or equivalent.
• Bachelor’s degree in Social Work or related field preferred.
• Strong understanding of patient navigation, specialty therapy access, and reimbursement.

Responsabilidades

• Lead, coach, and develop a team of field-based Patient Navigator Social Workers.
• Oversee patient access strategies and team productivity/quality metrics.
• Collaborate with patients, providers, specialty pharmacies, and internal teams to ensure timely therapy access.
• Guide on insurance coverage, prior authorizations, and financial assistance programs.
• Coordinate specialty pharmacy services, infusion therapies, and home-based transitions.
• Set and track individual and team performance goals with coaching plans.
• Support launches and improvements to patient access workflows and programs.
• Maintain accurate patient documentation and privacy compliance in CRM.
• Provide coverage to ensure staffing and continuity of care when needed.
• Work with field teams, market access, and providers to resolve access challenges.
