Qualifications

  • Maintain accurate patient documentation and privacy compliance in CRM.
  • Provide coverage to ensure staffing and continuity of care when needed.
  • Work with field teams, market access, and providers to resolve access challenges.

TalentBridge is partnering with a leading national healthcare organization to hire an experienced Lead Patient Navigator Social Worker to oversee a team of field-based Patient Navigator Social Workers supporting patients receiving specialty therapies.

The ideal candidate brings a strong background in specialty pharmacy, patient access, reimbursement, and healthcare operations, along with proven experience leading and developing teams.

Position Overview

The Lead Patient Navigator Social Worker will provide leadership, coaching, and operational oversight for a team of field-based Patient Navigator Social Workers. This individual will ensure patients and their families receive the education, resources, and support needed to navigate insurance coverage, reimbursement, financial assistance, and specialty therapy access.

This role combines people leadership, patient advocacy, reimbursement expertise, and cross-functional collaboration to improve patient outcomes and ensure the successful delivery of patient support programs.

Key Responsibilities
  • Lead, coach, mentor, and develop a team of field-based Patient Navigator Social Workers across multiple geographic territories.
  • Oversee patient access strategies and ensure team performance meets established productivity, quality, and service objectives.
  • Partner with patients, caregivers, healthcare providers, specialty pharmacies, and internal teams to facilitate timely access to specialty therapies.
  • Provide guidance on insurance coverage, prior authorizations, reimbursement challenges, and patient financial assistance programs.
  • Support the coordination of specialty pharmacy services, infusion therapies, and transitions to home-based treatment.
  • Establish individual and team performance goals, monitor results, and implement coaching and development plans.
  • Collaborate with program leadership to support new therapy launches, improve patient access workflows, and enhance existing patient support programs.
  • Maintain accurate patient documentation and case histories within CRM systems while ensuring compliance with applicable healthcare regulations and patient privacy requirements.
  • Provide direct patient navigation and operational coverage when needed to support staffing needs and maintain continuity of care.
  • Work collaboratively with field-based teams, market access professionals, and healthcare providers to resolve complex patient access and reimbursement challenges.
Required Qualifications
  • 10+ years of experience in the pharmaceutical or healthcare industry, preferably within specialty pharmacy, patient access, reimbursement, or patient support programs.
  • 3+ years of reimbursement management experience, including knowledge of insurance coverage, prior authorizations, specialty pharmacy, and patient financial assistance.
  • 3+ years of direct people management experience, with a demonstrated ability to lead, coach, and develop high-performing teams.
  • Licensed Social Worker (LSW, LMSW, LCSW, or equivalent) or equivalent relevant professional experience.
  • Bachelor's degree in Social Work, Health Sciences, Healthcare Administration, Business, Public Policy, or a related field preferred.
  • Strong understanding of patient navigation, specialty therapy access, healthcare reimbursement, and patient support services.
  • Experience working with specialty pharmacies, healthcare providers, pharmaceutical manufacturers, and/or patient support HUB programs preferred.
  • Excellent leadership, communication, problem-solving, and relationship management skills.
  • Proficiency in Microsoft Office and experience utilizing CRM or patient case management systems.
Location, Schedule & Travel

Location: Remote / Work From Home

Territory: Candidates must reside within 45 minutes of a major airport and be available to travel across the Eastern, Central, Mountain, and Pacific territories.

Travel: Approximately 25% overnight travel required.

Schedule: Must have flexibility to provide coverage during operating hours of 9:00 AM–9:00 PM EST, including alternating coverage and additional operational support as needed.

Training: Must be available to attend three days of in-person onboarding and product training.

Candidates must have a dedicated, quiet home office environment suitable for confidential patient interactions.
