Job summary

The Columbus Organization, LLC is seeking a Full Time Care Coordinator to support individuals with IDD in Augusta, GA. You will coordinate services, gather assessments, and monitor Medicaid eligibility while maintaining documentation and compliance.

This field-based role requires travel within assigned counties and occasional after-hours availability to assist families in crisis. Strong communication and data organization skills are essential.

Qualifications

• Minimum bachelor’s degree in human service field required.
• Minimum two years of experience serving persons with DD/IDD or related disabilities.
• Two years in healthcare, habilitative/rehabilitative or related setting required.

Responsibilities

• Assists people with intellectual challenges to access and coordinate support in the community.
• Coordinates gathering information for the support plan.
• Arranges and obtains clinically pertinent assessments.
• Monitors Medicaid eligibility and assists with paperwork.
• Maintains documentation and ensures HIPAA compliance.
