CNS Healthcare in Detroit seeks an experienced Case Manager for Wayne County offices. The role focuses on coordinating integrated mental and physical health services, using trauma-informed approaches and motivational interviewing to support individuals with mental illness or related conditions.

The position offers some remote flexibility after onboarding, with compensation ranges based on licensure and education level. CNS Healthcare provides a comprehensive benefits package and generous PTO.

Qualifications

• Qualified Mental Health Professional (QMHP) required.
• Bachelor’s Degree in Social Work, Sociology, Counseling, Psychology, Criminal Justice, or other Human Services field.
• One year of experience treating or working with a person who has mental illness OR specialized training.
• Valid and unrestricted Driver’s License.
• Transportation or automobile accessibility with current auto insurance.
• Proficient in Microsoft Office Suite and experience with Electronic Health Records (EHRs).
• Ability to communicate effectively and work collaboratively.

Responsabilités

• Provide community-based case management and coordinate care.
• Develop Person-Centered Plans aligned with funding and accrediting entities.
• Maintain thorough documentation in CNS policy and HIPAA standards.
• Gathers and interprets information to develop treatment plans and interventions.
• Coordinate medication management with primary and specialty care providers.
• Facilitate weekly groups and provide transportation assistance as needed.
