Job summary

Center for Life Management (CLM) operates the area’s most comprehensive outpatient mental health services for adults and elders. The Integrated Care team provides Medication Training and Support and wrap-around chronic disease management to reduce premature mortality among clients with mental illness.

Qualifications

• Bachelor’s degree preferred (nursing, psychology, social work, clinical mental health counseling).
• Associate’s degree with two years direct service may qualify.
• Ability to exercise sound clinical judgment in routine, urgent and emergent situations.
• Proficient computer skills with Microsoft Office applications.
• Ability to document effectively and efficiently in clinical work.
• Ability to navigate an electronic health record.
• Ability and desire to work as part of a clinical team with minimal supervision.
• Ability to acquire and practice new clinical skills as needs evolve.
• Effective communication, organizational skills and multi-tasking.

Responsibilities

• Provide counseling and Functional Support Services (FSS) in home or community settings.
• Provide Individual Therapeutic Behavioral Services (TBSI) to develop coping strategies.
• Provide Family Training and Counseling (FTC) to educate natural supports and promote recovery.
• Provide Medication Training and Support (MTS) to integrate prescribed meds into daily routine.
• Provide Crisis Intervention (CI) during acute symptom episodes for safety.
• Collaborate with the treatment team to ensure wrap-around care.
• Maintain set hours of billable service weekly and potential incentive pay.
• Participate on internal and external committees as directed by supervisor.
• Maintain confidentiality and professional conduct for all patients.
• Document services per agency policies and standards.
• Maintain professional memberships, licenses, and certifications.
• Represent CLM professionally in all settings.
• Perform other duties as assigned.
