Job summary

Center for Life Management (CLM) is seeking an Integrated Care Case Manager to provide care planning, resource identification, and service coordination for adults and elders.

The role includes delivering counseling and Functional Support Services (FSS) in home or community settings, and coordinating with other providers to ensure wrap-around care, with telehealth options where appropriate.

Qualifications

• Bachelor’s degree required; associate degree with relevant experience may qualify.
• Ability to exercise sound clinical judgment in routine, urgent, and emergent situations.
• Knowledge of chronic comorbid physical health conditions and experience with Choices for Independence (CFI) services.
• Proficient in Microsoft Office; ability to document efficiently in clinical work.
• Ability to navigate an electronic health record and work in a team-based setting.
• Strong organizational, communication, and multi-tasking skills.

Responsibilities

• Provide care planning, resource identification, and service coordination.
• Deliver counseling and Functional Support Services (FSS) in home or community settings.
• Provide Individual Therapeutic Behavioral Services (TBSI) to help coping strategies.
• Coordinate with internal and external providers for wrap-around care.
• Document all services per agency policies and standards.
• Participate in committees and uphold professional standards.
