Job summary

Center for Life Management (CLM) is hiring a Collaborative Care Case Manager to support treatment planning, benefit coordination, and community-based counseling. This full-time role engages with clients to improve symptoms and daily functioning through a wrap-around approach with internal and external partners.

Qualifications

• Bachelor's degree required; associate’s degree plus two years of direct service may qualify.
• Experience working with individuals with intellectual disabilities and/or traumatic brain injury preferred.
• Proficient with Microsoft Office and documenting in EHR.
• Ability to work in a clinical team while also being self-directed and motivated with minimal supervision.
• Strong communication and organizational skills and ability to multi-task.

Responsibilities

• Provide assessment to create treatment and care plans specifying problems, objectives, interventions and outcomes.
• Assist clients with applying for and maintaining medical coverage for access to care.
• Provide outreach, coordination, resource identification, linkage and monitoring of supports and services.
• Deliver counseling and Functional Support Services (FSS) in home or community settings as appropriate.
• Provide Individual Therapeutic Behavioral Services (TBSI) to develop and reinforce coping strategies.
• Provide Family Training and Counseling (FTC) services to educate natural supports.
• Provide Crisis Intervention (CI) during acute symptom flare-ups to ensure safety.
• Maintain professional documentation and compliance with agency policies and standards.
• Collaborate with treatment team, natural supports and external providers for wrap-around care.
• Participate in consultations, meetings and committees as required.
