Qualifications

• Bachelor's degree preferred or an associate’s degree with a minimum of two years of direct service in a related field.
• Proficient with Microsoft Office applications and able to document effectively in clinical work.
• Ability to navigate an electronic health record and work as part of a clinical team.

Responsibilities

• Develop care plans with assessment to set objectives and monitor outcomes.
• Assist clients with applying for and maintaining medical coverage for access to care.
• Coordinate outreach, resources and monitoring of supports and services for clients.
