Job summary

Vaya Health is seeking an Innovations Care Manager (Innovations CM) to provide proactive intervention and coordination of care for eligible members across MH, SUD, I/DD, and other health domains. This mobile role requires residency in North Carolina and travel to support in-home and community-based services.

You will work with care teams to plan and monitor interventions, reduce gaps, and ensure coordinated care in diverse settings.

Qualifications

• Experience coordinating care across behavioral health, medical, and LTSS domains.
• Knowledge of HIPAA and privacy practices.
• Ability to perform Health Risk Assessments (HRA) and other screenings.
• Strong communication with members, families, and providers.
• Willingness to travel and serve in diverse communities.

Responsibilities

• Ensures identification, assessment, and person-centered care planning for members.
• Links members with appropriate services across health domains (medical, behavioral health, LTSS).
• Meets with members to conduct the HRA and gather information on health and social needs.
• Administers screenings and provides education and self-management strategies.
• Supports development of a Care Plan addressing mental health, substance use, medical and social needs.
• Participates in multidisciplinary care team meetings and care planning.
• Provides crisis intervention and Transitional Care Management as needed.
• Coordinates Diversion efforts to prevent institutional placement.
