Partners Health Management in North Carolina seeks a Mobile Care Manager RN to deliver proactive intervention and care management for individuals with behavioral health, intellectual/developmental disabilities, and/or traumatic brain injury, ensuring high-quality, whole-person care.

The role involves collaboration with care management teams, scheduling and leading clinical huddles, reviewing data, and guiding education and treatment plans while monitoring outcomes.

Qualifications

• RN licensed in North Carolina.
• Two years of experience in IDD, TBI, and/or behavioral health.
• Willingness to travel regularly.
• Bilingual Spanish/English preferred.

Responsibilities

• Provides physical health expertise and consultation.
• Serves as the point of contact regarding collaborative care for care management teams.
• Uses data to provide proactive consultation and advice to care managers.
• Provides direct care management support as needed.
• Participates in clinical huddles/rounds with care managers and leaders.
• Takes lead on scheduling, presenting and updating in clinical huddles.
• Facilitates medication reconciliation.
• Supports staff development and training for improved member health.
• Guides use of Healthwise topics for education and promotion.
• Assesses outcomes and quality of care, identifying improvements.
