Job summary

Vaya Health seeks an Innovations Care Manager (Innovations CM) to provide proactive intervention and coordination of care for eligible members. This mobile role serves in members' home communities across NC counties, collaborating with care teams to align services across MH, SUD, I/DD, TBI, physical health, and LTSS, while maintaining HIPAA compliance.

Qualifications

• Experience conducting Health Risk Assessments (HRA) and creating person-centered care plans.
• Ability to link members to appropriate formal/informal services across medical and behavioral health domains.
• Familiarity with HIPAA requirements and patient privacy.
• Experience participating in multidisciplinary care team meetings and care management workflows.
• Ability to manage transitions and Diversion from institutional placement.

Responsibilities

• Identify and assess member needs and develop person-centered Care Plans.
• Coordinate services across medical, behavioral health, I/DD, and LTSS.
• Conduct HRA and document information in the Care Management platform.
• Lead or participate in multidisciplinary team meetings and care planning.
• Support Transitional Care Management and Diversion efforts for high-risk individuals.
