Job summary

Vaya Health is seeking an Innovations Care Manager to provide proactive intervention and coordination of care for members across MH, SU, I/DD and other health needs. This mobile role requires residency in NC and travel with in-person contacts in communities served.

The CM will conduct HRAs, administer screenings, and develop person-centered Care Plans with the care team, including coordination with RN and pharmacist.

Qualifications

• Meets with members to conduct the HRA and gather information on their overall health, including behavioral health, medical and social needs.
• Administers PHQ-9, GAD, CRAFT, ACES, LOCUS/CALOCUS and other screenings within scope and uses results for education and care linkage.
• Supports care planning to include mental health, medical and social needs and ensures plan meets NCDHHS requirements.

Responsibilities

• Identify, assess and plan person-centered care for members within a multidisciplinary team.
• Link members to formal and informal services across health domains and coordinate care.
• Update Care Plans annually or with significant life changes and participate in care team meetings.
