Centene Corporation seeks a registered nurse to develop and coordinate complex care management for primarily physical needs members in Georgia. The role focuses on personalized care plans, education for members and families, and collaboration with healthcare providers to achieve high-quality, cost-effective outcomes.

Qualificações

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2–4 years of related experience.

Responsabilidades

• Evaluates member needs and barriers to accessing care and coordinates care plan.
• Develops ongoing care plans and collaborates with providers and community resources.
• Identifies barriers and provides care management interventions.
• Coordinates between member/family and care team for person-centered care.
• Performs follow up and monitors status to update care plans as needed.
• Provides education to members and families on disease processes and care options.
• Collects, documents and maintains member information for regulators.
