Centene seeks a Care Manager (RN) to develop, assess, and facilitate complex care management for members with physical needs, emphasizing personalized care plans and education for members and families.

The role includes telephonic, digital, home, and site outreach; coordinating with providers to ensure timely access to services and ongoing follow-up on care plan goals, with focus on member-centered care and education on options and benefits.

Competenze

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.
• RN licensure valid in state of Georgia or compact licensure.
• 2–4 years of related experience in care management or nursing.

About This Role

The Care Manager (RN) develops, assesses, and facilitates complex care management activities for members with physical needs, focusing on personalized care plans and education for members and their families.

Highlights

• Location: Remote, GA
• Schedule: Full Time, Day shift
• Residency: Must reside in Georgia

What You'll Do

Requirements

Education:

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.

Experience:

• 2 – 4 years of related experience required.

Licenses & Certifications:

• RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
