Ideal candidates hold a PharmD or B.S. in Pharmacy, Tennessee licensure, and experience in hospice or call-center settings. The position features a remote/hybrid schedule with evenings and rotating weekend coverage.

Qualifications

• PharmD or B.S. in Pharmacy from ACPE-accredited school.
• Active license in Tennessee; PA license acceptable; willingness to obtain additional licenses.
• Hospice/palliative care or oncology experience preferred.
• Experience in call center or remote clinical consultation preferred.
• Familiarity with PBM services preferred.

Responsibilities

• Provide expert guidance on pain and symptom management for end-of-life care.
• Collaborate with nurses and hospice staff to develop medication care plans.
• Educate healthcare professionals and caregivers on medication usage and adverse event prevention.
• Document clinical interventions and patient outcomes in electronic health records.
• Maintain patient medication profiles in accordance with HIPAA and regulations.
• Engage in weekend rotations and support team coverage as needed.
