Job summary

Dragonfly Health is seeking a Pharmacist to support hospice and palliative care patients from a remote/hybrid setting. You will provide real-time medication guidance, collaborate with nurses and caregivers, and ensure safe, effective drug use for end-of-life care.

The role requires active PA licensure (PA or TN) and PharmD or B.S. in Pharmacy with ACPE accreditation. Evening schedule with rotating weekends emphasizes patient-centered care and strong communication.

Qualifications

• PharmD or B.S. in Pharmacy from ACPE-accredited school
• Active pharmacy license in home state (Pennsylvania or Tennessee)
• Pharmacy licenses in other states or willingness to obtain additional licenses

Responsibilities

• Provide expert guidance on pain and symptom management for end-of-life care.
• Collaborate with hospice staff to develop cost-conscious medication care plans.
• Educate healthcare professionals and caregivers on medication usage and adverse event prevention.
• Document clinical interventions and outcomes in electronic health systems.
• Maintain patient medication profiles in compliance with HIPAA.
• Meet productivity benchmarks and participate in weekend rotations.
• Communicate effectively in remote or hybrid work environments.
• Engage with multidisciplinary care teams in a fast-paced call center setting.
• Support team learning and knowledge sharing.
• Participate in weekend rotations and team coverage as needed.
