Zusammenfassung

Cleveland Clinic Health System is seeking an LPN Hospice to provide skilled nursing care in patients' homes through the Home Care and Hospice program. You will service approximately six patients daily, coordinating with an interdisciplinary team to deliver compassionate, evidence-based care.

The role involves travel within the Greater Cleveland area, with daytime hours from 8:00am to 4:30pm and every 4th weekend and holiday rotations. Experience in hospice or home health is preferred.

Qualifikationen

• Graduate from an accredited practical nursing program.
• Current Ohio license as a Licensed Practical Nurse.
• One year of clinical experience as a professional nurse in an acute setting.
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and proof of auto insurance.
• Knowledge of JCAHO and Medicare standards and regulations.

Aufgaben

• Ensure continuity of care and coordinate discharge planning.
• Provide direct patient care under multiple levels of care.
• Collect and document objective and subjective patient data per agency policies.
• Report data to the directing RN and health care team to initiate and adjust the plan of care.
• Educate patients and families about hospice and homecare benefits.
• Maintain complete and up-to-date documentation of patient condition and care interventions.
• Coordinate services at discharge to ensure a seamless transition to residential settings.
• Be flexible in setting and revising daily priorities based on patient needs and resources.
