The Coordinating Center in Maryland seeks a Homeless Support Services Coordinator to join the Housing Initiative Program (HIP). You will provide person-centered case management, connect clients to housing resources, and document progress in HMIS and other databases.

You will travel throughout Montgomery County conducting home and community visits, coordinating with CES, HIP housing locators, and community partners to secure permanent housing and prevent homelessness.

Qualificações

• Bachelor’s degree in a human services related field is required.
• Two years of case management experience is required.
• Ability to travel throughout Montgomery County and conduct home visits is required.

Responsabilidades

• Provide person-centered case management and service coordination for HIP clients.
• Coordinate with CES, HIP Housing Locator and community partners to promote housing stability.
• Maintain documentation in HMIS and other databases.
• Support residents from homelessness to permanent supportive housing.
• Engage in ongoing outreach and collaboration with program recipients and community partners.
