CareStar, Inc. is seeking an Assessment Specialist to work with residents across Franklin and several Ohio counties. The role combines remote work with field visits to assess eligibility, develop care plans, and connect individuals with essential services.

Ideal candidates hold an active RN/LSW/LISW/LPN license and have experience in home and community-based services. The position offers a competitive salary and comprehensive benefits, with a mission-driven team.

Qualificações

• Active license in RN, LSW, LISW or LPN.
• 12 months of home and community-based services experience in past 5 years.
• Knowledge of Medicare, Medicaid and Ohio Home Care waiver programs desirable.
• Adheres to CareStar policies and procedures.

Responsabilidades

• Completes visits to assess eligibility and safety per guidelines.
• Documents interactions and interventions in applicant records per policy.
• Processes PEAT or related tools and documents per policy.
• Assists with community resource referrals as appropriate.
• Identifies and reports high-risk situations as required.
