Job summary

Praxis Health is seeking a Quality Improvement Manager to lead HEDIS performance and population health initiatives. This remote role supports Oregon, Washington, and Idaho residents and partners with the Director of Population Health to implement sustainable strategies.

You will lead a team focused on quality improvement, payer incentive programs, and EMR quality functionality, ensuring workflows and data capture support ongoing performance gains.

Qualifications

• Bachelor’s degree in healthcare administration, nursing, public health, health information management, or related field (preferred).
• 3–5+ years of experience in healthcare quality, HEDIS, population health, payer quality programs, or quality improvement.
• Experience managing or supervising a team.
• Strong knowledge of HEDIS measures, quality metrics, care gaps, and payer incentive programs.
• Experience with EMR systems and quality-related functionality, reporting, registries, and workflows (Cerner/Oracle Health preferred).
• Strong analytical, communication, and organizational skills.
• Ability to collaborate with clinical providers, operational leaders, IT/EMR teams, analytics teams, and executive leadership.

Responsibilities

• Lead HEDIS performance strategy, monitoring, gap identification, and improvement initiatives.
• Track and analyze performance across quality metrics to identify trends and opportunities.
• Develop interventions to close care gaps and improve outcomes.
• Lead a team supporting quality initiatives and payer programs.
• Serve as EMR quality expert to optimize workflows and reporting.
