Job summary

Innovative Systems Group in Chicago seeks a licensed clinician to deliver medical management and health education for government programs. The role includes assessments, care planning, and care coordination in members' homes and by telephone.

This contract position is 40 hours per week, typically Mon–Fri 8:00–4:30 CST, with 90–95% remote work and occasional home visits. Initial 6- to 12‑month term with potential extension or conversion to permanent employment.

Qualifications

• Licensed clinician (RN, LPC or LMSW) with applicable state licensure.
• 3 years direct clinical care or 2 years for LPC/LMSW license track.
• Proficient with Word, Excel and PowerPoint; data handling skills.

Responsibilities

• Conduct medical management and health education for government programs.
• Gather, analyze and provide data for regulatory reports.
• Perform assessments, care planning and coordination in homes and by phone.
• Represent the company to members and collaborate with staff and vendors.
