Job summary

Elevance Health is seeking a Health Program Representative II to serve as the first contact between Care Management programs and identified members. The role supports enrollment and engagement calls, explains program features, and directs inquiries to clinical staff.

The position offers hybrid/virtual flexibility with occasional in-person onboarding, training sessions, and opportunities for mentoring less experienced team members.

Qualifications

• HS diploma or equivalent.
• Minimum 6 months experience or equivalent background.
• Ability to use call scripts and multi-channel communication.

Responsibilities

• Handle inbound/outbound enrollment and engagement calls.
• Provide program information and emphasize features/value.
• Record call details and follow client requirements.
• Direct inquiries to clinical staff when needed.
• Initiate referrals and manage consults for Care Management participants.
• Gather information from hospitals, plans, physicians, patients, vendors.
• Maintain accuracy and multi-task across channels in a fast-paced environment.
• Mentor less experienced Health Program Advocates.
