Job summary

Elevance Health is seeking a Health Program Representative I to support member care via phone and digital channels. This hybrid/virtual role offers flexibility, with required in-person training and occasional office work as needed.

Key duties include gathering member data, guiding Care Management participation, recording call details, directing inquiries to clinical staff, initiating referrals, and coordinating with hospitals and physicians.

Qualifications

• HS diploma or the equivalent.
• Minimum of 6 months experience in a telesales or customer service; or any combination of education and experience which would provide an equivalent background.

Responsibilities

• Gathers and records appropriate member information via telephone.
• Encourages members to participate in Care Management programs and explains services.
• Records call details and uses scripts as needed.
• Directs member inquiries to clinical staff.
• Initiates referrals and manages consults from clinicians.
• Gathers information from hospitals, health plans, physicians, patients, vendors and other referrals.
• Contacts physician offices to obtain data.
• Must multi-task while handling calls, texts, and electronic queues.
