Job summary

Community Healthcare Network in New York City is seeking a Patient Navigator for the Health Justice Mobile to support patient engagement, registration, insurance verification, care coordination, and linkage to services.

The Patient Navigator works under the supervision of the Program Manager and serves as a key contact for patients accessing services through the mobile unit; this position is funded through the Brooklyn District Attorney’s Office and continued employment depends on funds.

Qualifications

• Experience in health care, mental health, community health or human services setting.
• Experience using an electronic medical record system and patient registration.
• Experience serving underserved or vulnerable populations.
• Commitment to patient confidentiality and regulatory compliance.

Responsibilities

• Greet, engage, and support patients accessing services through the Health Justice Mobile Program.
• Complete patient registration, intake, demographic updates, and required documentation in the electronic medical record.
• Conduct outreach and follow-up calls to encourage appointment attendance and linkage to services.
• Verify health insurance eligibility and connect patients to CHN staff for insurance questions.
• Schedule appointments and coordinate follow-up visits.
• Assist with patient flow on the mobile unit to ensure timely service delivery.
• Provide information about health, mental health, sexual health, and supportive services.
• Maintain accurate records and protect patient confidentiality.
• Collaborate with the Program Manager and staff to resolve barriers to care.
• Assist with preparation of program reports with patient service data.
• Perform other related duties as assigned.
