Job summary

Qualifications

• High school diploma or equivalent required.
• Associate degree or coursework in health, human services, public health, social work, psychology, or related field preferred.
• Experience in health care, mental health, community health, or human services setting required.
• Experience using an electronic medical record system and verifying health insurance preferred.
• Experience serving underserved, justice-involved, or vulnerable populations preferred.

Responsibilities

• Greet, engage, and support patients accessing services through the Health Justice Mobile Program.
• Complete patient registration, intake, demographic updates, and required documentation in the electronic medical record.
• Conduct outreach and follow-up calls to encourage appointment attendance, continuity of care, and successful linkage to services.
• Verify health insurance eligibility and coverage and connect patients to the appropriate CHN staff for assistance with insurance-related questions and concerns.
• Schedule appointments, coordinate follow-up visits, and help patients connect to CHN providers and community-based services.
• Assist with patient flow on the mobile unit to support timely, respectful, and efficient service delivery.
• Provide information about available health, mental health, sexual health, and supportive services in a clear and culturally responsive manner.
• Maintain accurate records, protect patient confidentiality, and comply with agency policies and applicable regulatory requirements.
• Collaborate with the Program Manager, clinical staff, and community partners to resolve barriers to care and support patient needs.
• Assist with preparation of program reports by maintaining accurate patient service, referral, and follow-up information.
• Perform other related duties as assigned.
