Job summary

Community Health Associates seeks a Health Care Navigator to provide 24/7 in-home and community support under clinical supervision, assisting with rehabilitation, care coordination, and family empowerment to reduce crises and hospitalizations.

You will document progress in the EHR, lead group activities, travel locally for home visits, and collaborate with the clinical team to implement individualized service plans for diverse members and families.

Qualifications

• High School Diploma or GED; 21+; fingerprint clearance required.
• Reliable transportation with Arizona driver’s license and insurance.
• Bilingual preferred.

Responsibilities

• Assist members in accessing and maintaining services for community integration.
• Address immediate needs to minimize crises and prevent hospitalization.
• Document progress within 24 hours using DAP format in the EHR.
• Collaborate with families and clinical team on member progress and discharge planning.
• Conduct home and community visits and travel locally using a personal vehicle.
