Job summary

Community Healthcare Network's Health Justice Mobile program seeks a Patient Navigator to support patient engagement, registration, insurance verification, and care coordination. You will be a key contact for patients accessing services on the mobile unit, working under supervision to connect individuals with CHN providers and community resources.

Qualifications

• High school diploma or equivalent required.
• Associate degree or coursework in health, human services, public health, social work, psychology, or a related field preferred.
• Experience working in health care, mental health or human services setting required.
• Experience using an electronic medical record system and verifying health insurance preferred.
• Experience serving underserved or vulnerable populations preferred.

Responsibilities

• Greet, engage, and support patients accessing services through the Health Justice Mobile Program.
• Complete patient registration, intake, demographic updates, and required documentation in the electronic medical record.
• Conduct outreach and follow-up calls to encourage appointment attendance and linkage to services.
• Verify health insurance eligibility and connect patients to CHN staff for assistance with insurance questions.
• Schedule appointments, coordinate follow-up visits, and connect patients to CHN providers and community services.
• Assist with patient flow on the mobile unit to support timely, respectful, and efficient service delivery.
• Provide information about available health, mental health, sexual health, and supportive services in a clear, culturally responsive manner.
• Maintain accurate records, protect patient confidentiality, and comply with agency policies and regulations.
• Collaborate with the Program Manager, clinical staff, and partners to resolve barriers to care.
• Assist with preparation of program reports by maintaining patient service, referral, and follow-up information.
• Perform other related duties as assigned.

Education

High school diploma or equivalent
