Job summary

Community Healthcare Network, Inc is seeking a Patient Navigator for the Health Justice Mobile program to engage patients, assist with registration, insurance verification, and care coordination. The role focuses on linking patients to services and ensuring smooth navigation of care options.

Under supervision of the Program Manager, this position is funded through the Brooklyn District Attorney's Office; continued employment depends on funding. Schedule includes late weekday and weekend shifts.

Qualifications

• High school diploma or equivalent required.
• Associate degree or coursework in health, human services, public health, social work, psychology, or a related field preferred.
• Experience working in a health care, mental health, community health, or human services setting required.
• Experience using an electronic medical record system, registering patients, and verifying health insurance preferred.
• Experience serving underserved, justice-involved, or vulnerable populations preferred.

Responsibilities

• Greet, engage, and support patients accessing services through the Health Justice Mobile Program.
• Complete patient registration, intake, demographic updates, and required documentation in the electronic medical record.
• Conduct outreach and follow-up calls to encourage appointment attendance and linkage to services.
• Verify health insurance eligibility and connect patients to CHN staff for assistance with questions.
• Schedule appointments, coordinate follow-up visits, and help patients connect to CHN providers and community services.
• Assist with patient flow on the mobile unit to support timely, respectful service delivery.
• Provide information about health and related services in a clear, culturally responsive manner.
• Maintain accurate records and protect patient confidentiality.
• Collaborate with staff and partners to resolve barriers to care and support patient needs.
• Assist with preparation of program reports and data entry.
