Job summary

Qualifications
- RN license in IL (or reciprocal) with 3 years direct clinical care.
- For LPC/LMSW, 2 years of clinical practice to obtain licensure.
- 3 years wellness or managed care experience with clinical issues.
- Knowledge of health and wellness marketplace and employer trends.
- Verbal and written communication skills with members, staff and vendors.
- Willingness to travel within the assigned territory.
Responsibilities
- Conduct medical management and health education programs for customers on government health care programs.
- Gather, analyze and provide data for regulatory reports.
- Perform assessments, care planning and care coordination in member/family homes and via telephone.
- Represent the company to members and coordinate with internal staff and external vendors.
