Job summary

Qualifications

• Active nursing license (RN) or equivalent clinical license in state of operation.
• 2 years direct clinical care experience.
• Strong verbal and written communication abilities.
• Analytical skills with medical data understanding.
• Ability and willingness to travel within assigned territory.

Responsibilities

• Coordinate care for members via telephonic and field-based settings.
• Conduct health screenings, assessments, and care planning.
• Engage members to educate on health programs and adherence to goals.
• Support clinical operations with provider and member activities.
• May perform home visits or facility visits for complex populations.
