Qualifications
- Current, valid, unrestricted license in the state of operations.
- 3 years’ wellness or managed care experience with clinical issues.
- Strong verbal and written communication; interface with staff/vendors.
- Analytical experience with medical data; proficiency in MS Office.
- Valid driver’s license and ability to travel within territory.
Responsibilities
- Provide care coordination to members via telephonic and field-based care management.
- Educate members on health care programs and address personal health goals.
- Complete health screenings, assessments, and care planning; may visit homes/facilities.
- Collaborate with clinical operations, providers and care teams; travel as needed.
